Applying to Heatherwood On behalf of the prospective resident, a family member or responsible party will submit an application for admission to Heatherwood. Once eligibility is determined, our admission coordinator will arrange a pre-admission meeting with the family members or primary caregiver and the prospective resident. During this meeting, the admissions coordinator will explain the details of our program, review financial information and discuss the prospective resident's history, personality and psychosocial and medical needs. Evaluating Resident's Needs To help us develop a care plan tailored to each prospective resident's needs, we require a comprehensive in-home evaluation. This evaluation enables our team of professionals to determine the prospective resident's functioning levels and cognitive abilities. Comprehensive Care Our activity director and registered nurse review the care plan for the new resident and to further discuss what to expect from Heatherwood. Moving Day To assure that the new resident will receive the attention he or she needs during the initial adjustment period, we ask family members to contact their Heatherwood admissions coordinator to schedule a mutually convenient move-in date and time. As part of our welcome, the new resident and his or her family will tour Heatherwood and be introduced to the staff members who will care for the resident. The Transition Family members may wish to join the new resident for a meal in our dining area or spend some private time together to help him or her become accustomed to the new environment. Staying Involved We encourage family and friends to remain involved through volunteer programs and structured activity time. Our educational and support groups are available to our residents' families and friends and to the public. Visiting Hours Family and friends are always welcome and encouraged to visit Heatherwood daily between 9 am and 7:30 pm.
